Waiver of Subrogation - Changes to how BHHC calculates charges

The charge for a Waiver of Subrogation is now based on Manual Premium from base rates, instead of Estimated Annual Premium with net rates.  This creates consistency among all states in the basis of calculation.

In California, a Blanket Waiver is 2% of premium with a $350 minimum, and Project ("Specific") Waivers are 5% of attributable premium with a $350 minimum.  Other states' percentages and minimums will vary.

Waivers of Subrogation can now be included in quotes or still be added by Endorsement.
